MV. EVER ONWARD”.
This research assesses the impact of severe weather (typhoon) on the damage to
safety equipment on the main deck of the MV. Ever Onward and the subsequent corrective
measures. Using a descriptive qualitative method focused on a case study of the incident
during voyage 014A, the findings identify two primary causes: first, the lack of crew
preparedness in executing equipment securing procedures, and second, the suboptimal
condition and maintenance of the safety equipment, rendering it inadequate to withstand
the severe weather. Analysis using a Fishbone Diagram suggests solutions that include
enhancing specialized typhoon drills, optimizing checklists, implementing stricter routine
inspections and maintenance, and revising procedures within the Safety Management
System (SMS) to prevent the recurrence of similar damage in the future.
