Gas Dream.” A liquid carryover incident occurred in the cargo refrigeration compressor system during the tank stripping process on board MT. Gas Dream, a liquefied gas carrier. This incident disrupted the refrigeration operation and posed a serious risk of damage to the compressor, a critical component in the cargo handling system. Based on the investigation, the root causes were identified as the watch officer’s insufficient technical and procedural understanding of proper stripping operations, inadequate monitoring of critical parameters such as pressure, temperature, and liquid level, and inconsistencies in the application of the Standard Operating Procedures (SOP). The impact of this incident was significant, including delays in cargo handling operations, increased risk of mechanical damage to essential equipment, and financial losses due to operational downtime. However, the quick response from the Chief Officer and the Chief Engineer successfully mitigated further damage and restored control over the situation. This event highlights the importance of improving crew competence and reinforcing a strong safety culture on board. To prevent similar incidents in the future, it is recommended to implement standardized training programs both on the job (OJT) and off the job enhance monitoring systems through disciplined use of checklists and adherence to SOPs, and cultivate effective communication and leadership that promotes crew independence and accountability in maintaining operational safety.
